What Is a Surgical Abortion?
A surgical abortion, also called suction abortion or vacuum aspiration, is a safe and effective in‑clinic procedure used to end an early pregnancy. This method is typically a one-day procedure during the first trimester (up to 12 weeks of pregnancy).
During the procedure, the cervix (opening to the uterus) is gently dilated, and pregnancy tissue is removed using a small, flexible suction device.
- Procedure time: 5–10 minutes
- Total visit time: Up to 2–3 hours
- Anesthesia: Local anesthesia with optional IV sedation
Most patients experience cramping during and shortly after the procedure, followed by light bleeding or spotting that can last from a few days to a few weeks.
Safety and Risks
Surgical abortion is a very safe medical procedure, with rare complications that may include:
- Infection
- Heavy bleeding
- Injury to the cervix or uterus
- Incomplete abortion
- Continuing pregnancy

What to Expect During a Surgical Abortion Visit
Your visit will take approximately two hours. Below is a step-by-step overview of the process.
You can also explore our guide comparing Medication Abortion vs. Surgical Abortion to determine the best option for you.
Arrival and Check-In
When you arrive:
- You will check in at the front desk
- Our clinic prioritizes privacy and discretion
- Patients are not identified publicly as seeking abortion services
After completing paperwork and payment, you will move to a private room to begin your consultation.
Consultation and Medical Review
You will meet with a health educator and physician to:
- Review your medical history
- Discuss your pregnancy options
- Learn about the procedure, risks, and aftercare
- Sign a consent form
An ultrasound will be performed to confirm gestational age.
You will also receive:
- Cervical preparation medication (misoprostol) if 10–12 weeks pregnant
Medication Waiting Period (If Needed)
If misoprostol is given, you will wait about 90 minutes for it to take effect. You may relax in the waiting area during this time.
Abortion procedure
For first-trimester procedures (up to 12 weeks), we typically allow your support person to be right next to you for the whole procedure.
During the procedure:
- A speculum is inserted to view the cervix
- The cervix and vagina are cleaned
- Local anesthesia is applied
- The cervix is gently dilated
- A thin suction tube is inserted
- Pregnancy tissue is removed with gentle suction
The suction portion usually lasts about two minutes.
You may feel cramps similar to menstrual cramps, especially toward the end of the procedure.
Recovery After a Surgical Abortion
After the procedure:
- You will rest briefly in a recovery area
- Mild cramping and bleeding are normal
- Comfort items (heat packs, pads, snacks) are provided
Before leaving, you will receive:
- Aftercare instructions
- Antibiotics (if prescribed)
- Birth control options (if desired)
Returning Home
- You must have a ride home
- Rest for the remainder of the day
- Most patients resume normal activities the next day
Follow-Up Care
A follow-up appointment is not required unless you experience symptoms such as:
- Heavy bleeding
- Severe pain
- Fever
We recommend that you make an appointment with your regular physician for an annual Pap smear, a physical/gynecologic exam, and birth control refills.
You can contact the clinic anytime with questions or concerns.
